As a school community, FQMS and the PTSA want all activities to be available to all students. If your child will be unable to participate in a school activity or Intramural without financial assistance, please complete and submit to the principal the following form to request scholarship funds made available through the school and the PTSA.

Bricks are added to the Garden only once per year – typically near the school’s year end.  An updated form is typically distributed in March and due in May for that year's newest bricks.
